About the role
We are seeking a suitably qualified Registered Nurse to provide short-term contracted services supporting our integrated diabetes screening and assessment programme across our GP practices for over two months.
This role will undertake diabetes and cardiovascular risk assessment, provide tailored diabetes advice and education, support wh?nau self-management, and contribute to integrated follow-up and care planning within a broader primary care diabetes screening and assessment programme.
Start Date: Immediately.
Duration: Two months with flexibility on hours, but open to hearing any availability from June to August.
Essential requirements
Demonstrated experience in diabetes assessment, advice, education, and management.
Experience in primary care and/or long-term conditions management.
Ability to carry out CVDRA/CVRA and provide practical risk-reduction advice.
Strong communication, documentation, and team collaboration skills.
Desirable
Familiarity with ACDN / diabetes nursing professional frameworks.
Experience working with M?ori, high-needs populations, or in wh?nau-centred/community services.

Core service delivery requirements
- Carry out diabetes-focused nursing assessments and identify clinical and self-management support needs.
- Complete or support CVDRA/CVRA assessments using approved NZ tools/processes and communicate results clearly to patients/wh?nau.
- Provide tailored advice and education on:
- diabetes and pre-diabetes
- blood glucose monitoring / HbA1c understanding
- healthy eating and physical activity
- medication adherence and understanding treatment plans
- cardiovascular risk reduction
- when to seek follow-up or escalation of care.
- Support appropriate follow-up, referral, and escalation to GPs or specialist services where concerns are identified.
- Deliver care in a way that is culturally safe, wh?nau-centred, and equity-focused.
